Position Summary | Obtains required pre-admission information for the patients. Testing may include history and physical, laboratory work, EKG, radiology, and other pertinent testing. Reviews results with appropriate physician/provider(s). May make pre- and post- visit calls to patients. | | | |
Education | Associates degree in nursing is required. | Bachelor’s degree in nursing is preferred. |
Experience | 2 years staff RN experience is required. |
Certificates, Licenses and Registrations | Licensed Registered Nurse (RN) in the state of practice is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules. | Basic Life Support (BLS) is required. |

Responsibilities | Obtains all required patient data no later than three working days prior to scheduled procedure; keeping within timeframes required for this information/data. | Confirms procedures and times with other areas involved. Contacts physician's office about history and physical and other required tests. | Contacts patient to confirm appointment for history and physical and required tests if needed. | Delivers all required data no less than 24 hours prior to procedure/surgery. | Places pre-procedure patient telephone assessment calls within the appropriate time frame. | Places post-procedure follow-up telephone calls to patients within appropriate timeframe. | Documents all pertinent information on patient record accurately and timely. | Accepts telephone and verbal orders. | Collaborates with other health care professionals, to meet the healthcare needs of the patient. | Reviews current literature and shares with other staff. | Reports for work in a punctual manner, ready to perform professional duties. | Recognizes the individualism of every patient; treats each individual with compassion and promotes trust. |
Possess the information, skill, and understanding of the work to be performed. Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate. | Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable. Utilizes standard precautions during patient contact and when handling contaminated materials. Demonstrates compliance with National Patient Safety Goals. | Complies with all standards, policies and procedures including dress code, attendance and punctuality. Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues. | Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work. Actively involves the patient and family in decisions about care. Promotes quality care by participating in organizational councils/committees and/or support of related process. Documents services provided to patients in the electronic medical record in accordance with policy and requirements. |
