Position Summary

A resident pharmacist provides competent and compassionate patient care by working effectively as a member of the healthcare team. The resident will meet the educational goals according the American Society of Health-System Pharmacists' standards, as well as abide by the institution specific regulations. Teaching and mentoring pharmacy students, co-residents, and other multi-disciplinary colleagues allow for opportunities to develop life-long learning skills. 


Education

Doctorate in Pharmacy is required.


Experience

Competent in delivery of healthcare services for adolescents, adults and geriatric patients.


Certificates, Licenses and Registrations

Licensed Pharmacist in state of practice is required or obtained within 120 days.                   

IL: If not currently licensed as a Pharmacist in state of practice, then required to be licensed/registered as a Pharmacy Intern or as a Pharmacy Technician in state of practice. May work in a Pharmacy Technician license pending status for up to 60 days.

WI: Eligible for Pharmacist licensure in state of practice.

Responsibilities

Verify medication order entries for accuracy of dose, route, frequency and appropriateness through patient profile review & clarify provider orders when necessary.

Performs clinical pharmacy services as assigned and documents activates in the medical record as appropriate, i.e. nutrition support, oncology, pharmacokinetic dosing, audits, anticoagulation monitoring, and antibiotic monitoring.

Collaborates with other health care professionals to plan, monitor, review, and evaluate the quality and effectiveness of drugs and drug regimens, and monitor appropriateness of therapy based on patient status, lab work, and culture results.

Verify unit dose cart filing, parenteral product preparation, stock replacement, crash cart drawer replacement, and order filing by technical colleagues.

Prepare IV products requiring aseptic technique such as adult and pediatric hyper alimentation, chemotherapy agents, syringes, IV additives, irrigations, and mini-bag preparation.  Extemporaneously compounds items not readily available from a manufacturer.

Oversees pharmacy technicians, pharmacy intern(s) and pharmacy students.

Executes a plan for own professional development to expand competency in accordance with accepted professional standards.


Possess the information, skill, and understanding of the work to be performed.  Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate.

Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable.  Utilizes standard precautions during patient contact and when handling contaminated materials.  Demonstrates compliance with National Patient Safety Goals.

Complies with all standards, policies and procedures including dress code, attendance and punctuality.  Attends required in-services and meetings and completes all mandatory competencies by deadline.  Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues.

Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work.  Actively involves the patient and family in decisions about care.  Promotes quality care by participating in organizational councils/committees and/or support of related process.  Documents services provided to patients in the electronic medical record in accordance with policy and requirements.
