Position Summary

Provides competent, compassionate patient care, work effectively as a member of the health care team, and meet the educational goals of specific program. Teach and mentors junior resident/interns and medical students. Acts with integrity and honesty. Functions as an effective team member. Effectively manages conflict. Develops life-long learning skills. Attends all required educational conferences and participate in the planning of conferences per each program's requirements. Provides data on their educational experience to their program director. Carries out administrative responsibilities, such as dictation of discharge summaries, in a timely manner. Participates in medical center and departmental committees.


Education

Doctorate in Medicine is required.


Experience

Competent in delivery of healthcare services for adolescents, adults and geriatric patients is required.


Certificates, Licenses and Registrations

 Licensed in the state of practice is required.

Responsibilities

Completes History & Physical on all assigned patients within 24 hours of admission, develops and implements a plan of care with the attending hospitalists and treatment team and writes appropriate orders as per assignment of supervising attending. Provides direct patient care to ensure the safe, developmentally appropriate and effective delivery of treatment in accordance with accepted clinical standards as defined in departmental policies.

Completes individual daily rounds on all assigned inpatients and documents plan of care in medical record, rounding with the hospitalists as requested as per assignment of supervising attending. Provides routine medical management of patient care including ordering of diagnostic tests, labs, x-rays, medications and nutrition in compliance with regulatory agencies.

Implements age/developmentally appropriate clinical interventions including but not limited to meeting emotional, spiritual and psychosocial needs through involving patient in individual, group and family conferences, and documents these interventions in the medical record in compliance with regulatory agencies.

Manages discharge of patients with appropriate disposition, follow-up planning, adjunctive therapy management and education in conjunction with the multi-disciplinary treatment team and on all assigned inpatients documents & dictates a Discharge Summary in compliance with regulatory agencies.

Participates in the call rotation to provide coverage for the ER and the inpatient unit, responding to pages within 15 minutes as per assignment of supervising attending.

Provides supportive counseling for assigned patients, including the appropriate adjustment of mediations and documents progress in medical record in compliance with regulatory agencies.

As necessary, speaks directly with the Managed Care Company to appeal denials.

Collaborates with other attending physicians, multidisciplinary treatment team, social workers, colleagues from other departments, managed care groups and referral sources to coordinate the delivery of safe, developmentally appropriate and effective patient care in accordance with accepted clinical standards as defined in department policies.

Participates in multidisciplinary meetings and communicates treatment plans with Hospitalist, nursing, and treatment team, focusing on discharge needs.  Communicates with consulting physicians regarding patient’s treatment and discharge plan, especially as it relates to ongoing medication needs.

Demonstrates the ability to develop positive working relationships, utilizing effective communication with team members.

Participates in Performance Improvement activities, collects data on assigned Performance Improvement monitors, turns in reports on time and follows through on Performance Improvement action plans.

Performs as a professional within the Code of Ethics in the affiliate policy and in compliance with HIPAA and confidentiality rules and regulations, federal addiction confidentiality rules and regulations, and the Mental Health Code.


Possess the information, skill, and understanding of the work to be performed.  Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate.

Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable.  Utilizes standard precautions during patient contact and when handling contaminated materials.  Demonstrates compliance with National Patient Safety Goals.

Complies with all standards, policies and procedures including dress code, attendance and punctuality.  Attends required in-services and meetings and completes all mandatory competencies by deadline.  Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues.

Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work.  Actively involves the patient and family in decisions about care.  Promotes quality care by participating in organizational councils/committees and/or support of related process.  Documents services provided to patients in the electronic medical record in accordance with policy and requirements.
