Position Summary

Acts as liaison between allied health professionals and registered nurses in providing accurate and rational drug therapy to patients upon receipt of an allied health professional's order. Reviews order for dosing accuracy, drug selection, drug interactions, allergies, and appropriateness. Serves as a medical team consultant by participating in multidisciplinary rounds, committees, and workgroups. Provides drug information to allied health professionals, nurses, patients, and the general public.

 

 


Education

Pharmacy Required Meets educational requirements established in the state of practice

 

Graduate of accredited Pharmacy program; eligible for Pharmacist licensure in state of practice.

 


Experience

Residency preferred.


Certificates, Licenses and Registrations

Licensed Pharmacist in the state of practice Required or eligible for license.

 

Board Certification preferred.

Responsibilities

Collaborate with other health care professionals to plan, monitor, review, and evaluate the quality and effectiveness of drugs and drug regimens, providing advice on drug applications and characteristics

Interprets and verifies appropriateness of therapy and dosing range.

Process medication orders by reviewing patient profile

Verify medication order entries for accuracy.

Verify unit dose cart filling, parenteral product preparation, stock replacement, crash cart drawer replacement, and order filling by technical personnel.

Provide allied health professionals, registered nurses, and public with drug information such as drug selection, dosage, and proper administration.

Prepare IV products requiring aseptic technique such as adult and pediatric hyperalimentation, chemotherapy agents, syringes, IV additives, irrigations, and minibag preparation.

Clarify inappropriate physician orders when necessary.

Monitor appropriateness of therapy based on patient status, lab work, culture results.

Extemporaneously compounds those items not readily available from a manufacturer.

Oversee Pharmacy Technicians, Pharmacy Intern(s), and Pharmacy students

Performs clinical pharmacy services as assigned and documents activites in the medical record as appropriate, i.e. nutrition support, oncology, pharmacokinetic dosing, audits, anticoagulation monitoring, and antibiotic monitoring.

Executes a plan for own professional development to expand competency in accordance with accepted professional standards.


Possess the information, skill, and understanding of the work to be performed.  Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate.

Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable.  Utilizes standard precautions during patient contact and when handling contaminated materials.  Demonstrates compliance with National Patient Safety Goals.

Complies with all standards, policies and procedures including dress code, attendance and punctuality.  Attends required in-services and meetings and completes all mandatory competencies by deadline.  Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues.

Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work.  Actively involves the patient and family in decisions about care.  Promotes quality care by participating in organizational councils/committees and/or support of related process.  Documents services provided to patients in the electronic medical record in accordance with policy and requirements.
