Position Summary

The Manager-Home Care Services is responsible for oversight and leadership in day-to-day Home Health operations as well as managing staff. Assess patient satisfaction and resolves patient complains. Develops and enforces clinical practice guidelines and maintains policies for home health services.

 

 


Education

Bachelor’s Degree in nursing related discipline is required.

 

May consider a candidate who is currently enrolled in a Bachelor's of Nursing (BSN) program which must be obtained within 2 years of hire.

Master’s Degree in nursing related discipline is preferred.

 


Experience

3 years of progressively responsible leadership experience is required.

 

3 years nursing experience is required, preferably in Home Care.

 


Certificates, Licenses and Registrations

Licensed as a Registered Nurse (RN) in state of practice is required.

Responsibilities

Provides direct supervision of all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Maintains performance improvement CQI and quality control activities for department.

Maintains consistent effective communication through daily interaction/observation of work performance.

Reviews operating policies and procedures, recommends changes that will improve productivity and improve quality of care, and communicates to team as appropriate.

Leads colleagues using performance management and development processes that provide an overall context and framework to encourage colleague contribution.

Collaborates with others in providing excellence in patient care.

Works collaboratively with leadership to develop and implement a performance improvement program and address problems and/or complaints.

Participates in training and educational activities for personal development.

Coordinates interdepartmental activities within area of responsibility providing expertise and resources to ensure quality patient care.

Supports the team in addressing patient needs, responding to patient concerns and identifying ways to improve patient relations.


Leads a team of colleagues in human resource initiatives including but not limited to; performance management; education and development; selection and retention; workforce planning; coaching and counseling; and supporting activities that promote colleague engagement and satisfaction. 

Leads or supports the financial resource functions including but not limited to; managing expenses; preparing, monitoring and executing within limits of the budget; monitors productivity standards; manages the purchasing process and all decisions related to the finances for the area of responsibility. 

Leads or supports change and process improvement initiatives including but not limited to; strives for standard, efficient and value-added processes; communicates a clear vision; influences and motivates others; and develops strong relationships with key stakeholders.
