Position Summary | Provides leadership, clinical expertise and functional direction to team members in order to ensure the continuity of patient services. Serves as the expert resource and lead person within a service area. Works closely with the director/manager to plan, implement, and facilitate the work activity of the area to accomplish goals and objectives. | | | |
Education | Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required. | Bachelor’s Degree in Nursing is preferred. |
Experience | 3 years staff RN (Registered Nurse) is required. |
Certificates, Licenses and Registrations | Licensed as a Registered Nurse (RN) in state of practice is required. | Basic Life Support (BLS) is required. |

Responsibilities | Makes daily rounds on inpatients, coordinates care and acts as a liaison with physicians and all involved disciplines in order to oversee patient progression through the affiliate system and assists with answering questions relating to the patient’s transfer | Uses nursing judgment to determine the appropriate frequency and components of reassessment. Reassesses at regular intervals to determine patient response to interventions. Focuses ongoing reassessment on actual and potential problems and needs of the patient including education and discharge needs. | Follows nursing process of assessment, planning, intervention and evaluation, incorporating physical as well as psychosocial needs of designated patient populations. Performs comprehensive initial patient assessment and reassessments. | Provides and coordinates clinical program service education to the interdisciplinary team, including but not limited to nursing care, radiology, rehabilitation therapies, pharmacy, dietary, and care management. | Collaborates with other areas and/or disciplines and includes all stakeholders to develop methods and/or systems to identify opportunities for improving quality and decreasing costs. | Identifies and makes appropriate referrals for education/further assessment (i.e. nutritional services, spiritual care, diabetic educator, social services). Collaborates with other health care disciplines to plan interventions, patient outcomes and goals. | Develops teaching programs and provides education to patients and families based on their specific needs. |
Possess the information, skill, and understanding of the work to be performed. Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate. | Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable. Utilizes standard precautions during patient contact and when handling contaminated materials. Demonstrates compliance with National Patient Safety Goals. | Complies with all standards, policies and procedures including dress code, attendance and punctuality. Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues. | Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work. Actively involves the patient and family in decisions about care. Promotes quality care by participating in organizational councils/committees and/or support of related process. Documents services provided to patients in the electronic medical record in accordance with policy and requirements. |
