Position Summary | The LPN Physician Practice - Float is designated to float to clinics/departments based on the LPN's competency level. The LPN Physician Practice - Float is required to complete appropriate competency and cross-training to float to designated clinics/departments. Provides, assesses, plans, implements, and evaluates the care rendered to patients with urgent, emergent or potential health problems. Responsible for performing a wide variety of patient care activities for the clinic's patients of all ages, including general nursing care, administration of prescribed medications and treatments, education of patients and families, and response to patient telephone calls. All care is provided under the supervision of the physician and/or mid-level provider. | | | |
Education | Licensed Practical Nursing Degree is required. |
Experience | IV experience is preferred. | LPN experience is preferred. | Additional proficiencies may be required in some clinical areas. |
Certificates, Licenses and Registrations | Licensed as a License Practical Nurse (LPN) in the state of practice is required. | Basic Life Support (BLS) is required. | Advanced Cardiac Life Support (ACLS) is preferred. |

Responsibilities | Floats to designated clinics/departments within specialty area(s) based on competency level to provide clinical nursing care and support. | Participates in applying the nursing process under the supervision of the registered nurse. | Interacts with patients, family members, and other visitors courteously in an effort to promote customer satisfaction in accordance with standards and policy. | Documents patient care activities and patient outcomes in response to care promoting safe and effective nursing care in accordance with accepted nursing standards and departmental policy. | Participates in performance improvement activities. | Assists with meeting educational needs of the patient/family and distributes educational handouts when appropriate. | Is knowledgeable of basic laboratory test results and their implications for patient care. | Keeps RN and other appropriate persons informed of patient's status and contributes to patient plan of care. | Follows nursing process of assessment, planning, intervention and evaluation, incorporating physical, as well as psycho-social needs of designated patient populations, in person, and over the telephone. Practices within professional scope and current standards of care. | Documents according to professional scope and current standard of care ensuring that entries are legible and complete. | Assists in various aspects of patient flow process including, but not limited to, securing/stocking necessary supplies, preparing exam room and patient for examination, assisting the provider with the exam or procedure, providing treatments and medications as ordered by the provider and cleaning/disinfecting exam/treatment area after use. | Identifies initiates & assesses the effectiveness of known & potential patient problems and evaluates outcomes. Triages patients to the appropriate setting if there is a need to be seen. | Assists with various aspects of diagnostic testing including obtaining and/or preparing specimens, labeling/recording, and communicating results. Performs miscellaneous CLIA-waived testing which may include checks for throat pathogens, sexually transmitted diseases, urine dipsticks, and pregnancy tests; may perform EKG’s/rhythm strips. | Assists with preparing patients for physical exams (vital signs, history), review correspondences, collect incoming documents, write scripts as ordered or call the pharmacy, and address refills on medications. | Arranges for ambulance transport if required. | Is proficient with requirements for DOT physicals, urine and drug screens, and vaccinations. | Performs patient callbacks, reports diagnostic findings, provides consultation, scheduling of tests and procedures, and patient education. |
Possess the information, skill, and understanding of the work to be performed. Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate. | Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable. Utilizes standard precautions during patient contact and when handling contaminated materials. Demonstrates compliance with National Patient Safety Goals. | Complies with all standards, policies and procedures including dress code, attendance and punctuality. Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues. | Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work. Actively involves the patient and family in decisions about care. Promotes quality care by participating in organizational councils/committees and/or support of related process.
