Position Summary

Coordinates daily activities in home health and hospice. Supports continuity of care and maintains effective communication between staff and customers. Assists with staffing patterns, communication, supply management, patient quality and patient safety. Ensures that care and service are delivered and documented appropriately.


Education

Associates degree in nursing or related clinical field is required.

Bachelor’s degree in nursing or related clinical field is preferred.


Experience

3 years of clinical experience is required.

1 year of demonstrated leadership experience is preferred.

Prior experience in home health or hospice is preferred.


Certificates, Licenses and Registrations

Illinois: Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license is required.

 

Wisconsin: Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license is required. Basic Life Support (BLS) is required. Wisconsin:  RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.

Valid state driver’s license, current automobile insurance and reliable automobile is required.

Responsibilities

Manages incoming clinical calls, performs phone triage and assists callers using nursing judgment, critical thinking skills and informed decision making.

Completes aggregation and trending of assigned performance/patient outcome measuring systems.

Communicates pertinent information regarding patients, physician and staff to the home care supervisors/clinical manager.

Demonstrates adaptability to changing healthcare trends and shares knowledge and experience.

Assists with staffing, visit assignments and on-call, if needed.

Supports interdisciplinary team services to include coordination, reimbursement and clinical issues.

Assists clinicians in establishing immediate and long-term therapeutic goals, setting priorities and developing plan of care.

Works in conjunction with RN field staff to review OASIS documentation and offer suggestions or recommendations as appropriate upon patient admission.

Reviews patient medical records for service delivery and documentation compliance according to federal, state, local and regulatory agency policies/guidelines on a daily basis.

Maintains available, up-to-date patient/family teaching materials.

Completes quarterly clinical chart reviews for team or as needed.

Coordinates organization, maintenance, inspection and cleaning of equipment.


Possess the information, skill, and understanding of the work to be performed.  Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate.

Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable.  Utilizes standard precautions during patient contact and when handling contaminated materials.  Demonstrates compliance with National Patient Safety Goals.

Complies with all standards, policies and procedures including dress code, attendance and punctuality.  Attends required in-services and meetings and completes all mandatory competencies by deadline.  Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues.

Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work.  Actively involves the patient and family in decisions about care.  Promotes quality care by participating in organizational councils/committees and/or support of related process.  Documents services provided to patients in the electronic medical record in accordance with policy and requirements.
