Position Summary

The Community Health Outreach Specialist coordinates community health programming within respective assigned community. Collaborates with department leadership to develop, execute and evaluate initiatives based on Community Health Needs Assessment. Uses data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. Participates in local and regional programming aligned with community health needs assessment priorities to develop and manage health education programs that help individuals, families and communities maintain healthy lifestyles.


Education

Bachelor’s degree in public health, health education, epidemiology, or community health is required.

High School diploma, plus five years' experience in public health, health education, epidemiology, or community health may be considered in lieu of a Bachelor's degree.


Experience

3-5 years of experience in either health education, public health, or community-based leadership is required. (This experience is in addition to any experience in lieu of degree).

Knowledge, Skills and Abilities

Knowledge of chronic disease risk reduction strategies, disease processes and interventional procedures.

Knowledge of Social Determinants of Health as an indicator for long-term health outcomes and the guiding principals of Trauma Informed Care (TIC).

Knowledge of program evaluation and analysis.

Demonstrates strong verbal, written and listening skills.

Excellent organizational skills with ability to work with diverse groups and situations.

Develops and delivers effective presentations.

Responsibilities

Develops, executes, and evaluates initiatives based on Community Health Needs Assessment.

Implements programs and services for community outreach and health education in the primary and secondary markets.

Functions as an HSHS liaison between community healthcare partners, physicians, public health, and other engaged community organizations and HSHS Hospital represented.

Represents HSHS Hospitals on community coalitions, collaborative and committees as appropriate.

Assists with development, coordination, and execution of local and regional health events to include: Health Fairs, School-Based Health Education Programs; Community-Based Health Education Programs; Clinical-Based Health Education Programs; Community Benefit Strategies.

Supports annual IRS regulatory & system reporting requirements. i.e. Community Benefit Inventory for Social Accountability (CBISA).

Leads local Community Health Needs Assessment (CHNA) & Community Health Implementations plans (CHIP) in local region.

Supports triennial Community Health Needs Assessment (CHNA) & Community Health Implementation plan (CHIP) reports for division ministries.

Integrates the hospital's mission and vision into daily tasks through dedication to customer satisfaction, quality improvement, and collaborative working relationships.

 


Possess the information, skill, and understanding of the work to be performed.  Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate.

Abides by all safety requirements including demonstrating proper hand hygiene and wearing of Personal Protective Equipment (PPE), if applicable. 

Complies with all standards, policies and procedures including dress code, attendance and punctuality.  Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains ongoing communication and collaboration with team members.
