Position Summary | Performs and/or coordinates patient care in area of expertise utilizing the nursing process in collaboration with physicians and other healthcare providers along the continuum of care. Demonstrates advanced nursing skills through assessment, diagnosis and treatment of complex patients in area of specialty. Communicates the plan of care in area of expertise to other members of the interdisciplinary team. Provides care for patients of all ages according to the standards of care specific to the individual's needs. Primary duty includes the performance of work requiring advanced knowledge, in a field of science or learning, and which requires the consistent exercise of discretion and judgment. | | | |
Education | Master’s degree in Nursing is required. | Bachelor’s Degree in Nursing PLUS 5 years relevant experience may be considered in lieu of degree. | Doctoral degree in Nursing is preferred. |
Experience | Experience in management of patients across the care continuum preferred. |
Certificates, Licenses and Registrations | Licensed in the state of practice as Advanced Practice Registered Nurse is required. | Wisconsin Department of Safety and Professional Services (WDSPS) or | Illinois Department of Financial and Professional Regulation (IDFPR) | Basic Life Support (BLS) certification is required. | Board certification from the National Certification Corporation is preferred. | Controlled Substance license in state of practice is preferred. | Federal DEA license is preferred. | Neonatal Resuscitation Program (NRP) certification is preferred. |

Responsibilities | Orders medications, treatments, durable medical equipment and additional interdisciplinary involvement as needed. Orders and interprets laboratory and radiological tests. | Collaborates with and communicates summary of consultation recommendations/plan to referring physician and interdisciplinary team through progress notes and verbal communication. Coordinates, communicates, and advocates patient and family plans and goals of care among the interdisciplinary team and across the health care continuum. | Recommends and facilitates service improvements based on affiliate mission, service data and clinical research recommendations. Coordinates and evaluates quality improvement initiatives. | Follows nursing process of assessment, planning, intervention and evaluation, incorporating physical as well as psychosocial needs of designated patient populations. Performs comprehensive initial patient assessment and reassessments. Implements the plan of care including procedures, treatments, physician orders, nursing orders, education plan and medications. Coordinates implementation of plan of care with the multidisciplinary team members. Identifies and makes appropriate referrals for education/further assessment (i.e. nutritional services, spiritual care, diabetic educator, social services).Collaborates with other health care disciplines to plan interventions, patient outcomes and goals. | Develops, implements, and evaluates comprehensive consultation services in collaboration with the medical director or designated sponsoring physician. Possess the information, skill, and understanding of the work to be performed. Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate. | Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable. Utilizes standard precautions during patient contact and when handling contaminated materials. Demonstrates compliance with National Patient Safety Goals. | Complies with all standards, policies and procedures including dress code, attendance and punctuality. Attends required in-services and meetings and completes all mandatory competencies by deadline.
