Position Summary | Under the direction and supervision of the Registered Nurse, provides direct and non-direct patient care according to their scope of practice. Act as a versatile member of the care team, providing clinical support and a comforting presence to help achieve patients highest level of wellness. Assigned preceptor duties as needed. | | | |
Education | High School Diploma or GED is preferred. |
Experience | 1 year nursing assistant or Patient Care Assistant experience; or 6 months nursing assistant or Patient Care Assistant experience and certification as indicated below. |
Certificates, Licenses and Registrations | Certified Nurse Assistant (CNA) or Patient Care Technician (PCT) in the state of practice is required with 6 months experience as noted above. Certification is not required for those with greater than one year experience. | BLS (Basic Life Support) is required within 30 days upon hire. |

Responsibilities | Provides high quality, patient focused care, under the direct supervision of a Registered Nurse/ Performs delegated tasks in the patient care setting including obtaining vital signs, intake and output, daily weights, bathing, feeding, oral hygiene, ambulation, and repositioning. | Performs purposeful hourly visits including updating the communication board, and answers call lights promptly. Addresses the needs of patient and family. Reports patient and family concerns, needs, observations, and patient change of condition promptly to the RN. | May perform venipuncture, EKG’s and bladder scanning. Discontinues IVs, checks off blood products, and removes foley catheters. | Leads orientation, preceptorship, actively educates and demonstrates policy, procedures, and equipment use and mentoring of students and new colleagues. | Participates in the admission, transfer, and discharge process including patient room set up, orientation to unit, and managing patient belongings. | Accurately completes documentation in the Electronic Medical Record real time. | Recognizes and appropriately responds to safety concerns and at-risk behavior. Competent in performing all general and advanced level patient care tasks within their scope. | Performs point-of-care testing and specimen collection. Appropriately follows the policy and procedure outlined for specimen collection. | Completes various tasks such as emptying garbage, stocking supplies, transporting patients, delivering/picking up meal trays, and cleaning work area. |
Possess the information, skill, and understanding of the work to be performed. Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate. | Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable. Utilizes standard precautions during patient contact and when handling contaminated materials. Demonstrates compliance with National Patient Safety Goals. | Complies with all standards, policies and procedures including dress code, attendance and punctuality. Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues. | Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work. Actively involves the patient and family in decisions about care. Promotes quality care by participating in organizational councils/committees and/or support of related process. Documents services provided to patients in the electronic medical record in accordance with policy and requirements. |
