Position Summary | The Physical Therapist II–Home Care completes comprehensive physical therapy assessment in patient's home. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility. The Physical Therapist II–Home Care works cooperatively with physicians, case managers, and adjustors. Documents and reports on patients' progress. Additionally, Physical Therapist II–Home Care monitors, documents, reports patients' progress and serves as an educator for patients, families, students, co-workers, and self-professional advancement. | | | |
Education | Graduate (e.g., bachelors, masters, or doctorate) from an accredited Physical Therapy program which meets the licensure requirements in the state of practice is required. |
Experience | 2 years of Physical Therapy experience is required. |
Certificates, Licenses and Registrations | Illinois: Licensure or Eligibility for licensure as a registered physical therapist by the State of Illinois under the condition that they have been issued a letter of authorization from the Illinois Department of Financial & Professional Regulation (IDFPR) allowing them to practice under supervision is required. | Wisconsin: Licensure or Eligibility for licensure as a registered physical therapist by the Physical Therapy Examining Board in Wisconsin under the condition that they have been issued a temporary license allowing them to practice under supervision is required. | Certified Basic Life Support (BLS) or completion in the first 90 days of employment required. | Valid state driver’s license, current automobile insurance and reliable automobile is required. | Board Certification (e.g., CHT, OCS, etc.) which meets rigorous standards of maintenance or certification is preferred. |

Responsibilities | Assists with the overall management of the physical therapy department. For example, such responsibilities include, but are not limited to, maintaining inventories and requisitioning supplies, evaluating space requirements, and requisitioning repairs for equipment and facilities. | Completes comprehensive physical therapy assessment in patient's home and develop plan of care. Provides, modifies, or discontinues physical therapy modalities based upon an ongoing assessment/reassessment of the patient's clinical status and in accordance with provider orders. | Establishes an ongoing evaluation of patient’s status and goals, modifying treatment program for timely and enhanced progression. | Enhances patient education by developing comprehensive and progressive home programs with instruction to the patient, family, and other caregivers as appropriate to facilitate a safe and effective discharge. Prepares for patient discharge inclusive of ordering of necessary equipment and recommendations for therapeutic services ensuring the continuum of care. | Identifies and manages various levels of care necessary to ensure care plan delivery. Delegates and supervises the utilization of Assistants, if applicable, as well as support personnel to maximize patient outcomes and ensure effective utilization of services. | Utilize approaches to optimize physical and psychosocial needs based on experience, research findings, external resources and interdisciplinary collaboration and consultation. | Facilitates active involvement and contribution of others in team meetings and discussions regarding patient care. | Participates in mentorship (e.g., student/peer) to enhance quality and skill which includes acting as a resource for staff. Also, included is participation in professional organization or community volunteerism that engages professional representation. | Exceptional patient service and clinical quality reported by patient and/or demonstrated by functional improvement. Includes assisting in the development of training and/or clinical guidelines for a specific procedure, equipment, and/or program. Also, leads or participates in quality improvement or cost containment project, committee, or community event. |
Possess the information, skill, and understanding of the work to be performed. Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate. | Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable. Utilizes standard precautions during patient contact and when handling contaminated materials. Demonstrates compliance with National Patient Safety Goals. | Complies with all standards, policies and procedures including dress code, attendance and punctuality. Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues. | Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work. Actively involves the patient and family in decisions about care. Promotes quality care by participating in organizational councils/committees and/or support of related process. Documents services provided to patients in the electronic medical record in accordance with policy and requirements. |
