Position Summary | The System Director-Accountable Care provides strategic leadership for the development of hospital and provider centric assessments and performance improvement strategies, road map, tools, and methodologies. Develops and implements performance improvement capabilities designed to enable Strategic, Business and Clinical Transformation across the System and Clinical Integration Network (CIN). Provides leadership in the development and implementation of cross-continuum transformation plans to achieve targeted improvement goals. The role also serves at the key stakeholder leader responsible for understanding and integrating key plans around evolving payment models. | | | |
Education | Bachelor’s degree in Business Administration, Economics, Health Administration, Nursing or related field is required. | Master’s degree in Business Administration, Health Administration, or related field is preferred. | Associate's degree in Business Administration, Health Administration, Nursing or related field plus 3 years experience in healthcare leadership may be considered in lieu of Bachelor's degree |
Experience | 5 years leadership experience is required. | 5 years in nursing, provider or value based care experience required. |

Responsibilities | Key business leader who oversees the organization's accountable care programs and is responsible for oversight, support, and maintaining these programs, as well as enhancing population health initiatives in service of value-based care principles and outcomes | Leads the implementation of accountable care strategies within the system and provider organizations and provides work stream oversight for professional level engagement delivery staff from across multiple teams. | Oversees the care coordination, quality, and analytics leaders in developing and implementing integrated models of care management and quality improvement programs and services that control medical costs, create clinical efficiencies, and improve beneficiary health | Applies their understanding of clinical practice, healthcare policy and financing, risk-based provider relationships, and evolving reimbursement models and integration strategy to promote partnerships with employed and independent providers, third-party specialized vendors, community organizations and other key ACO stakeholders | Accountable for achieving business results in areas of responsibility within the accountable care department and in support of HSHS’s strategic goals.
