Position Summary

The Senior Accountable Care Performance Advisor will work closely with clinicians and their office staff, leveraging strong communication, relationship management and performance improvement skills, to achieve established quality and financial goals. This position serves as the Enterprise Population Health (EPH) accountable care performance primary point of contact for independent physician practices. In partnership with population health services organization team members, provides outstanding customer-focused service.


Education

Bachelor’s degree in Business Administration, Health Administration, Nursing or related field is required.

Master’s degree in Business Administration, Health Administration, or related field is preferred.


Experience

5 years value-based care and population health experience is required.


Certificates, Licenses and Registrations

Certified Professional in Healthcare Quality (CPHQ) is preferred.

Responsibilities

Educate clinicians and office staff on value-based care programs. Support practices on infrastructure development, shared resources, and standard work processes to assist with reduction of cost and improved quality throughout the care continuum. Serve as a value-based care subject matter expert to practices.

Educate independent clinicians and office staff on point of care and performance management tools embedded in supported EMRs.  Serve as the primary relationship manager, monitoring, maintaining and continually improving the relationship with independent clinicians and their practices.

Monitor independent clinician performance and meet regularly with providers and staff to provide updates on quality, utilization management and financial performance, including opportunities for improvement.

In partnership with independent providers and their staff, utilize available data, to develop performance improvement plans; utilize the plan do study act (PDSA) problem-solving model as needed.

Serve as a resource link for independent clinicians and practices to the necessary population health services operations, including credentialing, provider relations, referrals and claims processing.

Actively engage in identifying and responding to trends and opportunities within independent practices and recommend executable strategies.

Assist with the development of marketing materials outlining success within contracts.  Conducts monthly education webinars for independent physicians and providers.

Provide education about the incentive programs to physicians, including timely estimates of their performance as it relates to incentive opportunities.

Analyze practices’ data for compliance with network contracts and government reporting requirements. Coaches practices on how to meet requirements.

Collaborates with accountable care leaders to offer standardized services that are provided to independent physician practices to promote quality and cost reduction across the full continuum of care.

Engages in other Accountable Care projects as identified.

Keep abreast of industry standards and trends.


Possess the information, skill, and understanding of the work to be performed.  Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate.

Abides by all safety requirements including demonstrating proper hand hygiene and wearing of Personal Protective Equipment (PPE), if applicable. 

Complies with all standards, policies and procedures including dress code, attendance and punctuality.  Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains ongoing communication and collaboration with team members.
