Position Summary | The Team Lead-Advanced Practice Provider (APP) combines direct patient care with leadership responsibilities in support of accountable clinical and operational programs. The incumbent provides leadership for programs as well as recommendations for quality and process improvements. This position serves as a role model for the mission and values of HSHS, providing mentoring to Advanced Practice Providers to foster professional growth and clinical excellence. Additionally, this role serves as leadership representation on committees, councils and work groups including special projects to further HSHS and Physician Enterprise strategic initiatives. | | | |
Education | Master's or Doctoral degree in Nursing (for APRNs) or Physician Assistant Studies (for PAs) from an accredited program is required. |
Experience | 5 years clinical experience is required. | Two years experience in clinical leadership, mentoring, quality improvement or program development is preferred. |
Certificates, Licenses and Registrations | Licensed in the state of practice as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) is required. | National Board Certification in the area of specialty is required. | Basic Life Support (BLS) certification is required. |
Knowledge, Skills and Abilities | Demonstrated ability to lead APPs to provide high quality, patient-centered care that supports the mission and values of HSHS. | Proven ability to engage colleagues and foster individual growth and team collaboration. | Ability to drive clinical performance and operational efficiency within budgetary limitations while maintaining the highest quality standards. | Strong communication skills with the ability to lead others toward meeting productivity and quality standards. | Knowledge of regulatory, compliance, ethical and professional standards impacting APP practice. | Provides outstanding patient care and is role model for other clinicians across Physician Enterprise. |
Responsibilities | Provides clinical leadership for APP practice, ensuring effective integration, coordination, and alignment of clinical activities within the designated area of responsibility. | Fosters a culture that supports HSHS quality initiatives, identifies and removes barriers to achieving high-quality clinical care, and ensures timely and accurate reporting of quality outcomes to internal and external stakeholders. | Takes proactive role addressing and improving patient experience issues, consistently supporting and upholding standards of care. | Continuously promotes high-quality patient care by modeling dignity, respect, teamwork, cooperation, and open communication, contributing to a productive environment. | Provides direct patient care within the area of board certification in accordance with the highest standards of medical and professional practice. | Contributes to the development and implementation of strategic initiatives and supports business development activities that advance organizational and clinical goals. | Assists in the development and implementation of standardized protocols, procedures, and workflows to optimize patient experience, provider efficiency, and clinical outcomes. | Provides guidance, mentorship, and proctoring to APPs and other providers to enhance productivity, professional growth, and practice development. | Ensures APPs receive ongoing training, onboarding support, development, and facilitates coordination of medical staff activities with all relevant departments to support high-quality, integrated care. | Actively participates in population health initiatives and supports training and development related to effective population health management. | Obtains and provides education on population health practices and participates in community education initiatives to support broader health promotion efforts. | Communicates APP clinical performance relative to established goals and collaborates with other department leaders to coach and support APPs who are not meeting performance expectations. Collaborates with practice managers and collaborating physicians to perform performance evaluations for APPs. |
Possess the information, skill, and understanding of the work to be performed. Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate. | Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable. Utilizes standard precautions during patient contact and when handling contaminated materials. Demonstrates compliance with National Patient Safety Goals. | Complies with all standards, policies and procedures including dress code, attendance and punctuality. Attends required in-services and meetings and completes all mandatory competencies by deadline. Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues. | Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work. Actively involves the patient and family in decisions about care. Promotes quality care by participating in organizational councils/committees and/or support of related process.
