Position Summary

The Coordinator-Structural Heart (RN) is responsible for coordinating and managing the care of patients undergoing structural heart procedures, including but not limited to transcatheter aortic valve replacement (TAVR), transcatheter mitral valve repair (MitraClip), and left atrial appendage closure (WATCHMAN). Serves as the primary clinical liaison for structural heart patients and providers, ensuring efficient operations, continuity of care, and regulatory compliance.


Education

Associates degree in Nursing is required.

Bachelor’s degree in Nursing is preferred.


Experience

3-5 years of cardiovascular nursing experience is required.


Certificates, Licenses and Registrations

Licensed as a Registered Nurse (RN) in the state of practice is required.

Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.

Basic Life Support (BLS) is required.

Advanced Cardiac Life Support (ACLS) is required.


Knowledge, Skills and Abilities

Knowledge of structural heart disease processes and transcatheter therapies, structural heart program standards, CMS coverage requirements, and national registry participation requirements.

Demonstrates excellent communication, critical thinking, and problem-solving skills with the ability to collaborate effectively with physicians, staff, patients, and families.

Strong organizational and project management skills with ability to track multiple patients, timelines, and regulatory requirement simultaneously.

Strong analytical skills to monitor program metrics, registry reporting, and patient follow-up compliance.

Ability to perform high quality patient care and provide services within the scope of practice.

Ability to serve as a clinical role model and professional resource to other members of the team.

Ability to build and maintain professional relationships with physicians, both internal and external to the hospital/health system.


Responsibilities

Coordinates all aspects of care for structural heart patients from referral through post-procedure follow-up.

Works collaboratively with quality and IT to ensure TVT registry is maintained and that data related to valve patients is reported accurately and appropriately to meet regulatory requirements.

Coordinates and facilitates multidisciplinary case reviews and heart team meetings. Leads Structural Heart Council with physician partner.

Collaborates with revenue cycle teams to ensure appropriate payer authorization, coverage verification, and documentation to support reimbursement for structural heart procedures. Monitors program financial and operational metrics and supports leadership in tracking procedural volume, payer mix, and required follow-up to maintain regulatory compliance and program sustainability.

Partners and works collaboratively with physicians, nurses, leadership and other clinical staff to ensure high-quality care for structural heart patients.

Serves as primary point of contact for patients and families and liaison between hospital departments and referring providers.

Performs patient assessments including medical history review, risk stratification, and procedure eligibility screening.

Schedules diagnostic testing, clinic visits, procedures, and follow-up appointments.

Provides pre-procedure education and post-procedure discharge planning to patients/families.

Rounds on patients and families on day of procedure and keep families updated throughout procedure. Rounds on hospitalized patients to assist in assessment, coordination of care, and discharge planning.

Monitors patient outcomes and identifies complications or barriers to care.

Develops and implements educational materials in conjunction with the Medical Director, physicians, and department leadership to support the needs of the patient and family.

Assists in performance improvement initiatives and best practice implementation by conducting assessments of patient care and analyses of processes to identify opportunities and create action plans to address.

Assists in the development of policies, procedures, and workflows for the structural heart program in accordance with regulatory and national standards.

Participates in staff education and training related to structural heart procedures.


Possess the information, skill, and understanding of the work to be performed.  Actively participates in continuing education opportunities and maintains certifications, licensure as appropriate.

Reduces the risk of healthcare acquired infections through compliance with current CDC hand hygiene guidelines and facility policies including the wearing of Personal Protective Equipment (PPE), if applicable.  Utilizes standard precautions during patient contact and when handling contaminated materials.  Demonstrates compliance with National Patient Safety Goals.

Complies with all standards, policies and procedures including dress code, attendance and punctuality.  Attends required in-services and meetings and completes all mandatory competencies by deadline.  Commits to keeping all patient, student or colleague information confidential and maintains excellent interpersonal communication with the health care team colleagues.

Emphasizes a holistic approach to patient care-attending to body, mind, and spirit while upholding ethical principles with patient care and all other aspects of work.  Actively involves the patient and family in decisions about care.  Promotes quality care by participating in organizational councils/committees and/or support of related process.  Documents services provided to patients in the electronic medical record in accordance with policy and requirements.
